This interactive webinar shared the development of the Connect and CARE model for increasing patient-pharmacist engagement and led participants through the application of the concepts and tools. 

In Canada’s new pharmacy environment we recognize that pharmacists are looking for both new and more effective ways to engage with their patients for routine care and to offer new pharmacy services. This model was developed to help busy community pharmacists better connect with patients. Increasing engagement creates opportunities for patients to better understand and appropriately use community pharmacy services to realize optimal medication therapy outcomes.

05/2013  Connect and CARE: Engaging patients over-the-counter in Canada's new pharmacy environment

The profession of pharmacy continues to experience unprecedented change. As a result, models and tools are needed to support pharmacists transition the way they are caring for their patients.

The goal of this project was to produce an evidence-based model that would support community pharmacists to increase engagement with patients in community pharmacies. For this work the team conducted a scan of the academic and grey literature, interviewed stakeholders across Canada, and conducted focus groups with pharmacists and patients. This research provided the foundation for the development of the Connect and CARE model.
